Health and Welfare Overview:

 

Medical:

SuperGen offers two options for medical care, a PPO or HMO, to eligible employees and their eligible dependents.  The employee pays a small premium for medical expenses.

 

Dental/Vision:

These premiums are at no cost to the eligible employees and their eligible dependents.  SuperGen offers a wide range of providers both in and out of network.

 

Group Life Term Insurance and AD&D:

Eligible employees are covered at one times their annual salary.  We also provide coverage for eligible dependents.  Voluntary group term life insurance is also available at very competitive rates.

 

Short and Long Term Disability:

SuperGen's Short Term disability plan covers all eligible employees up to 60% of their salary, with a weekly maximum of $2,500.  This applies after a 14 day elimination period.  After exhausting the Short Term disability an eligible employee would be eligible for Long Term disability.  Long Term disability covers all eligible employees up to 60% of their basic monthly income up to a $10,000 maximum.

 

Flexible Spending Accounts (FSA’s):

SuperGen has two FSA plans, Medical Reimbursement Account and Dependent Care Account.  These plans allow employees to use tax free income to pay for certain non-reimbursable health and dependent care expenses. 

 

AFLAC:

SuperGen offers voluntary Cancer and Accident insurance through AFLAC. These plans are above and beyond our benefit plans.

 

Employee Assistance Plan (EAP):

SuperGen provides a confidential service to their employees and family members.

Investment Programs:

 

401(k) plan:

Participation in the SuperGen 401K plan is available after 2 full months of service.  Our plan is managed by Merrill Lynch.  The next quarter following your 2 months of service you may enter the plan.  SuperGen provides a matching contribution on a sliding scale up to 3% with an annual maximum of $6,000.

 

ESPP (Employee Stock Purchase Plan):

Employees are able to purchase SUPG common stock through payroll deductions at 85% of the fair market value on the Enrollment Date or Exercise date, whichever is lower. 

There are two offering periods beginning:

May 15th

November 15th

 

Time off Benefits:

 

Vacation:

All regular full time employees accrue three weeks of paid vacation per year.  Following their five year anniversary employees accrue four weeks of paid vacation time.

 

Sick/Personal Time:

All eligible employees accrue two weeks of personal/sick time annually.  We encourage employees to use this time for illness for themselves or their dependents, and for medical or other personal appointments.

 

Bereavement Time:

SuperGen offers bereavement leave to provide time to attend to a funeral, attend to funeral arrangements and/or personal matters for the loss of immediate and extended family members.  Depending upon the distance and closeness of the deceased, time off is granted for up to three days.

 

Company and Floating Holidays:

SuperGen provides a total of eight paid holidays per year.  In addition we provide three floating holidays at the start of each year.  These days can be used for any purpose.
